Refactor XmlNode and libxml usages.

- Use std::move to transfer ownership.
- Avoid libxml primitives outside XmlNode.
- Have attribute setting return errors.
- Mark bool returns with WARN_UNUSED_RESULTS and use return value.
- Use std::string over char*.
