Set default --segment_duration to 6 seconds 

This is recommended by Apple and other content providers:
https://apple.co/2uTTAc4

- Set default --clear_lead to 12 seconds, i.e. about 2 segments.
- Set default --fragment_duration to 0 so fragment (sub-segment) is not
  generated by default.
