HLS byte range

- Use byte range format if segment_template is not set in MediaInfo.
- Change HlsNotifier::NotifyNewSegment() to take start byte offset.
- Change HlsNotifyMuxerListener to cache the segment information if
  MuxerOptions::segment_template is set.
