- Add flags and stream descriptor fields for HLS.
- Remove fields from MuxerOptions. Instead pass them directly to
HlsNotifyMuxerListener.

- Attribute for EXTINF should be CODECS instead of CODEC.

SimpleHlsNotifier::NotifyNewStream().
