This feeds into efforts to create a working install target.
The order of headers is still funky, and was "fixed" by clang-format,
but in a way that doesn't exactly align with the style guide. Further
cleanup of header order is coming in a follow-up PR.

After this it should be possible to build a working `packager` application from the `cmake` branch. Some further logging improvements may be needed to get full parity with the `main` branch in terms of ability to do verbose debug logging, but other than that everything is expected to work.
This fixes our use of std::filesystem to interpret all paths names as
UTF8. Before this, UTF8 paths did not work correctly in all cases.
This also adds a new unit test to cover this case.
On Windows, it is critical that a UTF8 locale be set at runtime.
Applications linking with Packager as a library should call setlocale(),
and the Packager frontends now do this automatically after converting
wide character arguments into narrow strings.

The file_name fields will be used to solely indicate file paths on the
designated file system, and they are used to do normal file operations,
including file creation, file updating and file removal if needed;
added new xxx_url fields, for the URLs that should appear on DASH
manifest or HLS playlists.
xxx_url are the URIs of the media in the manifest. The fields are
converted from file_name fields but adjusted to be relative to DASH
manifest path or HLS playlist path, optionally with base_url prepended.
Previously the file_name fields are converted in place to indicate
URLs when passing to manifest / playlist builders. The original file
names were lost, which made it difficult to remove files outside of
live window.
Now that the input file names are preserved. File system APIs can
operate on the original file names while manifest / playlist generation
functions can operate on URLs.

The current behavior of logging to a file in the same directory
as the binary will break if the binary does not have write access
to that directory.

ThreadedIoFile spawns a new thread for every new file. Thread
information is stored for tracking purpose by base::tracked_objects.
The tracking object remains even if the thread itself is destroyed.
This results in memory usage increased by a couple of bytes for every
new segment created in live mode (new segments spawns new threads).
Use WorkerPool instead to avoid spawning new threads.

For mp4 outputs, a metadata box with version information is added to
moov box.
For mpd outputs, a text comment with version information is added in
the beginning of mpd file.
