Uses EXPECT_NEAR for comparison with an acceptable error bound. Also
increases error bound to 50ms.

Also rename mpd/base/bandwidth_estimator_test.cc to
bandwidth_estimator_unittest.cc for consistency.
And remove mpd/base/simple_vod_mpd_notifier.* as they are not used
anywhere.
