This converts all time parameters to signed, finishing a cleanup that
was started in 2018 in b4256bf0. This changes the type of:
- timestamps
- PTS specifically
- timestamp offsets
- timescales
- durations
This excludes:
- MP4 box definitions
- DTS specifically
This is meant to address signed/unsigned conversion issues on arm64
that caused some test cases to fail.

Instead, the actual earliest presentation time is used except for
the first segment if there is an offset between presentation time
(pts) and decoding time (dts).
Chrome (as of v66) reports dts instead of pts in buffered ranges in
MSE API. To avoid breaking Chrome, the earliest_presentation_time
of the first segment is set to its dts as Chrome does not like negative
values for
adjusted dts = dts + Period@start (0 for the first period)
- presentationTimeOffset (earliest_presentation_time).

Previously packaging completes successfully without any error
or warning.
With the fix, an error will be reported if write fails. It may
appear as "Cannot close file error" as we use threaded IO, which
could delay the error reporting until Close() call, so the user
of the File API needs to make sure Close() returns successfully.
Also fixed a deadlock in threaded_io_file if internal_file->Write
fails.

- Changed OnMediaEnd() to take (sub)segment ranges.
- Define a structure to pass the ranges.
- This requires muxers to "cache" the segment ranges but in most cases,
they already do it, e.g. 'sidx' for mp4.

- Add hls_name and hls_group_id fields to MuxerOptions. This is used to
pass the NAME and GROUP-ID values for EXT-X-MEDIA tag to
MuxerListener.
- Change MuxerListener::OnEncryptionInfoReady() to take an
initialization vector.
- Change MuxerListener::OnNewSegment() to take segment name.
- Reworded and formatted MuxerListener comments to Doxygen style.

The segment duration was not reset after segment was finalized. It
was reset after adding one sample. This new sample was added to the
new fragment but was not recorded in the new segment, which resulted
in off-by-one error in the segment generation.

Due to various libraries' inconsistencies with using EoF, this change
will instead continue to read a file until the number of bytes read is
0. It considers bytes_read == 0 to mean end of file.

Increases the read/write buffers for packager input and output files to
2MB from 256KB, which helps throughput when using network file systems.

CreateTemporaryFileInDir uses mkstemp which works only on local files.
This change enables the use of other types of files, like network file
as temporary directory.
